New Patients

When you join the Practice you will be asked to complete a registration form and a brief questionnaire about yourself. If you have never been registered with a GP in the UK before you will be asked to present your passport to confirm your ID and some form of proof of address.

NEW PATIENT CHECKS

You will be invited (by letter) for a New Patient Check with one of our nurses for all newly registered patients over 5 years.

This will be a ten minute appointment which incorporates a routine health check - please bring along a fresh urine sample to this appointment.
