Forms & Medicals

We are often requested to complete various forms for patients. These mostly take the form of insurance reports, health statements (i.e. fitness to fly) or passports. Occasionally a physical examination may be required (i.e. taxi license). These services are not part of the NHS and as such will usually incur a fee.  

If you have a form that requires completing please hand it in to the receptionist who will be happy to give you an approximate "verbal" quote or if you wish will arrange for a formal written one. We usually require one week to process these forms.
